Notice of Privacy Practices
Effective Date: October 5, 2026
THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our commitment to your privacy
Iron & Oak Longevity (“we,” “us,” or “the Practice”) understands that health information about you and your care is personal, and we are committed to protecting it. We are required by law to:
- Keep your health information private and secure.
- Give you this notice of our legal duties and privacy practices.
- Follow the terms of the notice that is currently in effect.
- Notify you if there is a breach of your unsecured health information.
How we may use and share your health information
We may use and share your information in the following ways without needing your written permission.
For treatment. We use and share your information to provide, coordinate, and manage your care. For example, we may share information with a pharmacy to fill your prescription, a laboratory to run your tests, or another provider, such as your primary care provider, who is also caring for you.
For payment. We use and share your information to bill for and collect payment for our services. Because we are a self-pay practice, this mainly means processing your card payment. If you ask for a receipt to submit to your own insurance, we will provide it.
For health care operations. We use and share your information to run our practice and improve the quality of our care. For example, we may review charts to assess the quality of our care, which may include review by a supervising physician or another qualified reviewer.
Appointment reminders and care communications. We may contact you by phone, text, email, or patient portal to remind you of appointments, share lab results or follow-up instructions, or tell you about treatment options or health-related services we offer.
Business associates. We may share information with outside companies that help us run the practice, such as our electronic health record, telehealth, and payment vendors. We require them to protect your information.
Other uses and sharing allowed or required by law
We may also share your information without your written permission in limited situations, including:
- When required by law.
- For public health activities, such as reporting certain diseases or problems with medications.
- To report suspected abuse, neglect, or domestic violence.
- To health oversight agencies, such as licensing boards, for audits and investigations.
- In response to a court order, subpoena, or other lawful process.
- To law enforcement, in limited circumstances allowed by law.
- To coroners, medical examiners, and funeral directors.
- For approved research, when privacy protections are in place.
- To prevent a serious threat to someone’s health or safety.
- For workers’ compensation and specialized government functions, as the law allows.
Uses that require your written authorization
We will get your written permission before we use or share psychotherapy notes (if any), use or share your information for marketing, or sell your information. Any use or sharing not described in this notice also requires your written authorization. You may cancel an authorization in writing at any time, except for actions we have already taken based on it.
Texas law may give you added privacy protections. When state law is stricter than federal law, we follow the stricter rule.
Your rights
Inspect and copy. You may ask to see or get a copy, on paper or electronically, of your medical and billing records. We will respond within the time required by law, and we may charge a reasonable, cost-based fee.
Request a correction. If you believe information in your record is wrong or incomplete, you may ask us to correct it. We may say no, and if we do, we will tell you why in writing.
Request restrictions. You may ask us to limit how we use or share your information. We are not required to agree, with one exception: if you pay for a service in full out of pocket, you may ask us not to share information about that service with your health plan, and we will agree.
Confidential communications. You may ask us to contact you in a certain way or at a certain place, such as by text only. We will say yes to reasonable requests.
Accounting of disclosures. You may ask for a list of certain times we have shared your information over the past six years.
Paper copy of this notice. You may ask for a paper copy at any time, even if you agreed to receive it electronically.
Breach notification. You have the right to be told if your unsecured health information is involved in a breach.
Choose someone to act for you. If someone is your legal guardian or holds your medical power of attorney, that person can exercise your rights for you.
To use any of these rights, contact our Privacy Officer using the information below. Requests for records, corrections, and restrictions should be made in writing.
Changes to this notice
We may change this notice, and the changes will apply to all health information we keep. The current notice will be posted on our website and available to you on request.
Questions or complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the federal government. We will not retaliate against you for filing a complaint.
